CBH Philadelphia: Open Network for Group Practices, Claims and Provider Relations

Community Behavioral Health is the HealthChoices behavioral health plan for Philadelphia, run as a division of the city's Department of Behavioral Health and Intellectual disAbility Services. For years it contracted mainly with agencies. On December 2, 2025 it opened its network to independent practitioners and group practices.

Short answer

Effective December 2, 2025, CBH accepts independent practitioners and group practices into its provider network. Eligible licenses are CRNP, LMFT, LPC, psychologist, psychiatrist, LSW, and LCSW; group members must be individually enrolled in PROMISe and share one license type; and practices need a physical location within 45 miles of Philadelphia County. CBH Provider Relations is at 215-413-7660.

Where CBH fits

CBH covers Philadelphia County under HealthChoices. Pennsylvania carves behavioral health out of HealthChoices. Each county, or group of counties, contracts a Behavioral Health Managed Care Organization, and the member's county of Medicaid eligibility decides which one you bill, regardless of their physical health plan. Confirm the member's county and BH-MCO in the state's Eligibility Verification System before every new episode.

Where claims go wrong

  • Mixed-license groups. A group TIN with psychologists and LCSWs together, which PA Medicaid enrollment does not allow under one group.
  • PROMISe enrollment gaps. A clinician billing under the group before their own PROMISe enrollment is active.
  • Companion guide mismatches. EDI files that ignore CBH's 5010 companion guide requirements.
