MDD ICD-10 Codes: F32 and F33 Major Depressive Disorder Code List (FY2027)

Major depressive disorder is coded on two axes: is this the first episode or a recurrence, and how severe is it now? F32 covers a single episode and F33 covers recurrent MDD, and each splits by severity, psychotic features, and remission. Getting the axis right is most of the work.

Short answer

The ICD-10-CM codes for major depressive disorder (MDD) are F32.0 to F32.5 and F32.9 for a single episode, and F33.0 to F33.3, F33.40 to F33.42, and F33.9 for recurrent MDD. Each is chosen by severity (mild, moderate, severe, with or without psychotic features) or remission status. F32.9 and F33.9 are the unspecified codes; F32.A is "Depression, unspecified" when MDD hasn't been established.

Billing pitfalls

  • F32.9 or F32.A on month six of therapy. Fine at intake, a review trigger later. Update the code once severity is assessed.
  • Single episode when the history shows recurrence. A patient with a prior episode belongs in F33. The intake history should say so.
  • Severity with nothing behind it. Document how severity was judged. A standardized measure such as the PHQ-9 helps support it.
  • Headers on claims. F32.8 and F33.4 aren't billable. Use F32.81, F32.89, or F33.40 to F33.42.
