Behavioral Health ICD-10 Codes: Depression, Anxiety, PTSD & ADHD (FY2027)

Every behavioral health claim pairs a CPT or HCPCS code with an ICD-10-CM diagnosis, and the diagnosis is where a surprising number of denials start: unspecified codes on ongoing treatment, Z codes listed first, codes that stop at a header. These are the codes therapy and psychiatry practices bill most, checked against the FY2027 code set.

Short answer

The most common behavioral health ICD-10-CM codes are F32.0–F32.9 and F33.0–F33.9 for major depressive disorder, F41.1 for generalized anxiety disorder, F41.0 for panic disorder, F43.10–F43.12 for PTSD, F43.20–F43.29 for adjustment disorders, and F90.0–F90.9 for ADHD. The FY2027 update, effective October 1, 2026, didn't change any of them. Code to the specificity the record supports; unspecified codes on ongoing treatment draw denials.

Billing rules that cause diagnosis denials

  • Unspecified codes are a placeholder. F32.9, F32.A, F33.9, F41.9, and F43.20 are valid, but many payers and Medicaid programs flag them on ongoing treatment. Use them at intake if needed, then update once the assessment supports a specific code.
  • Code to the highest specificity the record supports. Severity, episode, and remission status belong in the code when the note documents them.
  • The diagnosis must support the service. A code that doesn't fit the CPT code tends to deny as CO-11. See behavioral health denial codes.
  • Z codes rarely work as the primary diagnosis. Codes like Z63.0 (Problems in relationship with spouse or partner) describe circumstances, and many plans won't pay psychotherapy with one listed first.

Headers aren't billable

F32.8, F33.4, F43.1, F43.2, and F43.8 are category headers. A claim with one of them is missing a character and comes back as a CO-16 with remark M76. Use the full code, such as F43.10 rather than F43.1.

Pair these with the right procedure codes: behavioral health CPT codes and H codes.

Code titles from the FY2027 ICD-10-CM code set (CDC/NCHS), effective October 1, 2026 through September 30, 2027. Diagnosis is a clinical decision; this page covers how codes behave on claims, not how to diagnose.

Frequently asked questions

What is the ICD-10 code for depression?

For major depressive disorder, F32.0 to F32.5 (single episode) or F33.0 to F33.42 (recurrent), by severity and remission. F32.A is depression, unspecified, and F34.1 is dysthymic disorder.

What is the ICD-10 code for anxiety?

F41.1 for generalized anxiety disorder, F41.0 for panic disorder, F40.10 for social anxiety, and F41.9 for anxiety disorder, unspecified.

What is the ICD-10 code for ADHD?

F90.0 (predominantly inattentive type), F90.1 (predominantly hyperactive type), F90.2 (combined type), F90.8 (other type), or F90.9 (unspecified type).

Did behavioral health ICD-10 codes change in 2026?

Not the ones on this page. The FY2027 ICD-10-CM update took effect October 1, 2026, and left the depression, anxiety, PTSD, adjustment disorder, and ADHD codes unchanged.

Diagnosis and procedure codes that agree.

Stable checks each claim's diagnosis against the service and the payer's policy before it's sent, so CO-11 and M76 denials don't happen.
