Psychiatry E/M Coding: Billing E/M with Psychotherapy
Psychiatry is one of the few specialties that regularly bills an evaluation and management (E/M) visit and psychotherapy in the same encounter. Done right, it captures the full value of the visit. Done wrong, it's a reliable source of denials and downcodes.
The two-part visit
A medication-management visit that also includes therapy has two billable components: the E/M service (the medical evaluation and management) and a psychotherapy add-on for the therapy portion. You bill both, but each has its own rules.
Selecting the E/M level
Office E/M codes (99202–99215) are chosen based on medical decision making (MDM) or total time on the date of service. The key trap: when you bill a psychotherapy add-on, the time spent on therapy cannot also count toward time-based E/M selection. Use MDM for the E/M level in that case, or keep the time strictly separated.

Add-on codes are never billed alone, they accompany the E/M service.
Where psychiatry E/M billing goes wrong
- Double-counting time. Counting therapy time toward a time-based E/M level.
- Weak documentation. Notes that don't clearly separate the E/M work from the psychotherapy.
- Missing the add-on. Billing only the E/M and leaving the therapy revenue on the table.
- Interactive complexity. Forgetting 90785 when it applies.
