HT Modifier: Multi-Disciplinary Team, and How States Redefine It

HT has three meanings depending on where the claim goes. If your practice bills more than one state, this is the modifier most likely to be right in one and wrong in the next.

Short answer

HT is the HCPCS Level II modifier "Multi-disciplinary team." Michigan uses HT for mobile crisis services, previously TJ. Ohio Medicaid uses HT as a procedure modifier for a service rendered by a supervised trainee or assistant under the supervision of an LISW, LIMFT, LPCC, LICDC (SUD only), or licensed school psychologist, required only where direct supervision is priced differently.

Ohio's supervision pair

Ohio pairs HT with HP: HP for supervision by an MD, DO, psychologist, CNS, CNP, or PA, and HT for supervision by an independently licensed social worker, marriage and family therapist, counselor, chemical dependency counselor, or school psychologist. Both apply only where the rate differs for direct supervision.

Why HT claims deny

  • Generic meaning sent to Ohio. HT used for a team service, where Ohio reads it as licensed-supervisor pricing.
  • Mobile crisis without HT in Michigan. The crisis service is reported without the mobile crisis modifier.
  • Supervision undocumented. Ohio HT billed without the supervising practitioner and supervision in the record.
