H0020 Code Description: Methadone Administration (OTP) Billing
Opioid treatment programs bill H0020 more often than almost any other code, and almost every denial comes from the same place: the modifier that says whether the dose was given in the clinic or taken home.
Short answer
H0020 is "Alcohol and/or drug services; methadone administration and/or service (provision of the drug by a licensed program)." States set the modifiers that separate supervised from take-home doses. Ohio uses HF for daily administration and TV, UB, TS, or HG for one to four weeks of take-homes; Texas uses UA for facility doses and U1 for take-homes, limits U1 to 30 per 30 days, and denies H0020 with no modifier.
| Code | HCPCS descriptor |
|---|---|
| H0020 | Alcohol and/or drug services; methadone administration and/or service (provision of the drug by a licensed program) |
Ohio: daily and weekly administration
| Situation | Ohio billing |
|---|---|
| Patient seen daily for methadone | H0020 with HF; date of service is the dose date |
| Take-homes covering 1 week | H0020 TV |
| Take-homes covering 2 weeks | H0020 UB |
| Take-homes covering 3 weeks | H0020 TS |
| Take-homes covering 4 weeks | H0020 HG |
| Daily buprenorphine | T1502 with HF |
Once a weekly modifier is billed, the OTP cannot bill H0020 again in that period, and the date of service is the visit date.
Ohio also bars 96372 on the same date as H0020 or T1502 administration unless the appropriate J code is on the claim.
Texas: facility and take-home doses
- UA for supervised doses. Methadone administered with supervision in a facility.
- U1 for take-home doses. Billed for each date a take-home dose is dispensed, limited to 30 per 30 days by any provider.
- No modifier, no payment. H0020 submitted without a modifier is denied.
- Once a day. Outpatient methadone is limited to once per date of service by any provider, at a fixed daily rate.
- No 96372 that day. 96372 is denied for the same date of service as H0020 or H0033.
Colorado
Colorado's August 2026 manual lists a Medicaid directed payment rate of $22.44 for H0020 in opioid treatment programs.
Common H0020 denials
- Missing or wrong dosing modifier. The claim cannot show whether the dose was supervised or taken home.
- Overlapping periods. A daily H0020 billed inside a period already covered by a weekly take-home modifier.
- Injection code on the same day. 96372 billed alongside methadone administration.
