H0006 Code Description: Alcohol and Drug Case Management

H0006 is the SUD case management code: linking a person to housing, benefits, medical care, and recovery supports. It is also the one H code Ambetter adds in 2027.

Short answer

H0006 is "Alcohol and/or drug services; case management." Ohio bills SUD case management as H0006 in 15-minute units at $22.03 for every rendering practitioner type, with a practitioner modifier identifying who delivered it, such as U2 for an LPC, U4 for an LSW, U5 for an LMFT, U3 for an LCDC, and HM, HN, or HO for care management specialists by education. Ambetter begins covering H0006 on January 1, 2027.

Ambetter adds H0006 in 2027

Ambetter's notices that drop 48 H codes on January 1, 2027 also add H0006 as covered. See the full Ambetter change.

Common H0006 denials

  • Practitioner modifier missing. Ohio requires the modifier for non-independently licensed staff.
  • Case management versus counseling. Notes describe treatment rather than coordination and linkage.
  • Duplicate case management. Another agency or program already billing case management for the same person.
