CO-252 Denial Code: Documentation Required on Behavioral Health Claims
CO-252 isn't a no. It's a payer saying it won't decide the claim until it sees something: progress notes, a treatment plan, an assessment. Behavioral health gets these on the codes payers watch most. The claim pays if the records arrive in time and support it.
Short answer
Reason code 252 means "An attachment/other documentation is required to adjudicate this claim/service." With group code CO, the provider can't bill the patient while it stands. It always comes with a remark code naming what's needed, such as M127 (missing medical record) or N706 (missing documentation). Send exactly what's named, through the payer's attachment channel, with the claim number, before its deadline.
What behavioral health payers ask for
| Service | What's usually requested |
|---|---|
| 90837 | Progress notes showing start and stop times and why the longer session was needed |
| 90839 and 90840 | The crisis note: risk assessment, interventions, disposition, total time |
| 90785 (interactive complexity) | The note showing the communication barrier |
| IOP and PHP | Treatment plan, level-of-care assessment, daily attendance |
| H code services | Treatment or person-centered plan and service notes |
| High-frequency outpatient therapy | Treatment plan with goals and progress |
How to send it
- Use the channel the payer names. Portal upload, fax with a cover sheet carrying the claim number, or an electronic attachment (a 275 transaction, referenced by a PWK segment on the 837).
- Send what's named, all of it. A partial response is the most common way CO-252 turns into a denial for lack of documentation.
- Mark the deadline. Payers close the request if nothing arrives within their window, and many then deny the claim outright.
- Send attachments up front for services that always draw a request, if the payer accepts unsolicited attachments.
CO-252 vs. similar codes
CO-252 asks for records. CO-16 means the claim data itself was wrong. CO-50 means the payer reviewed the records and decided the service wasn't medically necessary. See all behavioral health denial codes.
Code wording from the X12 Claim Adjustment Reason Code and Remittance Advice Remark Code lists, read October 19, 2026. X12 updates the lists several times a year, and each payer decides which codes it sends for which problem, so read the remark codes on your own remittance.
Frequently asked questions
What does denial code CO-252 mean?
The payer needs an attachment or other documentation to decide the claim. A remark code names what, such as M127 for a missing medical record or N706 for missing documentation.
How do I fix a CO-252 denial?
Send the documentation named in the remark code through the payer's attachment channel, with the claim number, before the payer's deadline. Don't resubmit the claim without the records.
Is CO-252 a denial?
It's a request that stops payment until the records arrive. It becomes a final denial if nothing is sent in time or the records don't support the claim.
Which behavioral health codes get documentation requests most?
90837, crisis codes 90839 and 90840, 90785, IOP and PHP, H code services, and unusually frequent outpatient therapy.
